TESTAMENTARY DISPOSITION  - This is a will for your Shee Atika shares.  All shareholders are encouraged to complete this form; whether you have one share or 100 or more shares. 

GIFTING - To gift shares to your child, grandchild, great-grandchild, niece, nephew, brother, or sister.

DIRECT DEPOSIT - Enroll for direct deposit of your distributions and receive your distributions early!  Checks are mailed ten days after direct deposits are made.

NAME CHANGE - Has your name recently changed?  While it is not necessary to issue you a new stock certificate we would appreciate your updated information. 

RELEASE OF CUSTODIAN - Please fill out and submit this form if you are a shareholder or custodian of a shareholder who has turned 18.

DEATH OF A SHAREHOLDER - To transfer shares upon death of a shareholder according to Testementary Disposition or Will. 

LOST STOCK CERTIFICATE - If you have lost your certificate you may be issued a new certificate by filling out and submitting this form.
